We are please to announce that Collings Park has now completed the pilot for Plymouth EMIS Practices and have transitioned over from EMIS LV to EMIS WEB Medical Computer system. This will ensure that we stay at the "forefront" of IT Medical Technology and provide the highest standards of clinical and administrative support at all times for our highly valued patients.

About the Practice

The practice was established before the formation of the N.H.S. by a single-handed practitioner working from his house in Eggbuckland Road serving the then semi- rural areas of Compton and Hartley.  Some patients even remember cattle being driven along Eggbuckland Road.  With the building of new housing in Efford and Eggbuckland however, the practice has grown. Four doctors now practise in the present surgery which was newly converted and opened in 1990.

The team take great pride in providing a friendly and efficient family doctor service from our modern computerised surgery. Doctors, nurses, health Care Asistants, health visitor, midwife and administrative staff form the team which works closely together, not only to help you when you are ill, but also to try to prevent serious diseases from occurring.

A map giving this boundary in more detail is available in the surgery for inspection.  If you are moving outside the area and wish to stay with the practice please discuss this with the doctor/Practice Manager as it is not always possible to keep patients registered at the practice who move out of the Practice area.  

Registering with the Practice

New patients are very welcome.  To register with the Practice please report to reception where you will be given forms to complete your registration. You can also download these forms by clicking on the following links, print and complete; bring both forms with you when registering.
Health Questionnaire

You will require two forms of identification:

  • a passport or Full driving Licence (both parts)
  • a utility bill with your name and address on it

We will also need to confirm that you live within the Practice area.  We encourage all patients to complete a short health questionnaire and then make an appointment to see the Practice Nurse who will check your height, weight, urine and blood pressure (which can be high without symptoms).  If you are on medication or a medical opinion is required she will arrange a consultation with the doctor. You will be registered with the Practice as a whole and will be assigned to the list of one of the Doctors.  When making an appointment you may request a consultation with the Doctor of your choice.

Temporary Residents

We are always happy to see any relatives or friends staying with you if they need a Doctor or Nurse.  They will need to fill out a short form for the information to be returned to their own Doctor.  Please arrive 10 minutes before your appointment to allow time for completion of the necessary forms.

Out of Hours calls/emergencies (two calls required)

24 hour care is provided for Medical Emergencies

When the surgery is closed an answering machine is available on Tel No: 01752-771500. The answering machine provides patients with the details they require to contact the Out Of Hours Devon Doctors Service tele number (0845-6710-270) the out of hours number to call.  You are not able to leave a message on the answer machine. 

On calling the Out of Hours number the receptionist will take your number and the doctor will phone you back to discuss whether advice or a visit is appropriate.  There may be a delay if the doctor is out on another call. This service is for urgent medical situations – if you, or a member of your family, become ill and you are concerned.  It is not for routine enquiries, such as booking an appointment with your GP, repeat prescriptions, test results etc.

NHS Direct can give you general medical information 24 hours a day.  Their Trained Nurses can provide you with expert health advice and reassurance any time of the day or night.

Visit a local Pharmacy (chemist). Your local paper will have details of Pharmacies open late on weekdays and on Saturdays, Sundays and Bank Holidays.

If your injury or illness is severe, or you have had a serious accident, call 999 and ask for an ambulance.  Or you can visit your local A&E Department.  If you are calling on behalf of someone else please have relevant information available, such as:

  • address
  • date of birth
  • current medication
  • name of their GP

If considered necessary you will be referred to a duty Doctor or Nurse Practitioner who will contact you and, either give further telephone advice, or ask you to attend a local centre or in appropriate circumstances visit you at home.

NHS Direct offers free health information and advice 24-hours a day.  For deaf people and those hard of hearing, a telephone service is available on 0845 606 4647.  If English is not your preferred language you can choose to use a confidential translation service.
